The Home & Community Care programs are designed to support elderly and people with disabilities in their efforts to remain living as independently in the community as possible.

There are several different programs that are targeted to specific age groups and targeted to specific types of disabilities.

Available programs and services:

Information, Assessment and Nursing Home Relocation (PDFs)

The first step to access services is a call to our intake workers at 651-266-3613 for adults and 651-266-4500 for children. Over the phone we will explore with you what you think you might want or need; what types of services may be available; review eligibility criteria for various options, review the types of supports you may already have in place; and then determine together if a home visit to conduct an assessment is of interest. If interested in applying for services, an application packet for our Home and Community Based programs will be mailed to you.

Picture of the elderlyThe assessment, referred to as a Long Term Care Consultation, is done with a Social Worker or a Public Health Nurse who will come out and meet with you and anyone you wish to include. At the conclusion of the assessment, recommendations will be made for you to consider in determining your need for support. The waiver programs listed below each have some very specific eligibility criteria. You may choose if you meet the criteria, to participate in one of the waiver programs. You could also choose to find alternative ways to meet your needs.

Ramsey County, Minnesota Chemical Dependency Programs

 

Ramsey County's Chemical Dependency Services are part of a division of the Ramsey County Community Human Services Department offering a wide variety of services to help people of all ages who are experiencing various emotional and/or behavioral problems associated with chemical dependency or substance abuse along with co-occurring mental health difficulties.

Services available, depending upon need and eligibility:

Case Management provides information and referral resources; coordination of services; crisis intervention and community outreach as an alternative or adjunctive service to other types of chemical dependency treatment.

Case managers assess, with the client, family members, and other professionals, what services the client needs.

Case managers then arrange, coordinate and monitor services, and, in addition, provide some of these services directly.

Pre-petition Screening is provided for all individuals who are being petitioned for civil commitment, a process by in which the Court determines that an individual’s mental illness or chemical dependence is causing them to be a danger to themselves or others.

Individuals who are placed under civil commitment may be treated in a community or state hospital or receive intensive community based care during this time of acute need.

Detoxification is provided by the Ramsey County Detox Center for adults and adolescents, 24 hours per day, seven days a week; and assessment and referral to appropriate treatment programs.

The Detoxification Center is a sub-acute residential facility providing medical monitoring and chemical dependency assessments and screening for individuals who are intoxicated or experiencing withdrawal symptoms.

Chemical Use Assessments and Referrals to Chemical Dependency Treatment and other Services are provided for Ramsey County residents by the Chemical Health Unit.

Assessments of one’s use of alcohol or other drugs are provided on a no fee basis for eligible residents, or for a charge on a sliding fee scale based on income and number of dependents.

The Chemical Health Unit also serves as an access point for State licensed chemical dependency treatment services for qualified individuals meeting both State income and clinical eligibility guidelines for public funding.

Referrals to supportive Group Residential Housing for eligible persons with chemical dependency are also available.

Treatment resources for co-occurring mental health and substance abuse disorders are also available through the Chemical Health Unit. It is estimated that 60% of people who experience a mental illness also experience a co-occurring addiction to alcohol or drugs. Research has demonstrated the need to treat co-occurring disorders through integrated treatment.

An assessment may be scheduled by calling 651-266-4008 between 8:00 a.m. and 4:30 p.m.

Client Eligibility and Fees: The services of the Ramsey County Chemical Dependency Services Division are available to Ramsey County residents.

  • Chemical use assessments are provided on a no fee basis for eligible residents, or for a charge on a sliding fee scale based on income and number of dependents.

  • Treatment services are available for qualified individuals meeting both State income and clinical eligibility guidelines for public funding at no charge.

  • Temporary Group Residential Housing for eligible persons with chemical dependency may also be available at no charge.

  • Prospective clients with pre-paid health coverage should consult their health plan about chemical health services and coverage.

Financial Assistance Services

Financial Assistance Services provides support to low-income residents of Ramsey County. We determine eligibility for public assistance programs such as Medicaid, Food Support, General Assistance, Child Care and the Minnesota Family Investment Program.

Note: If you currently have Medicare or will qualify in the near future, you should learn about Medicare D, the new prescription drug coverage program. Go to Minnesota Department of Human Services for more information on Medicare-related programs.

Child Care Assistance Program (CCAP) - This program helps to pay childcare costs for low-income working families.

Minnesota Family Investment Program (MFIP) - This program is Minnesota's Cash and Food Assistance program for families who need assistance.

Diversionary Work Program (DWP) - When most families first apply for cash assistance, they will participate in the Diversionary Work Program, or DWP. This is a 4 month program that helps parents go immediately to work rather than receive welfare.

Food Support Program (FS) - This program helps low-income people with their food costs.

Minnesota Food Assistance Program (MFAP) - Non-citizens not eligible for FS may qualify for the MFAP.

General Assistance (GA) - This program serves as Minnesota's primary safety net for single adults and childless couples. The GA Program provides monthly cash grants for vulnerable persons whose income and resources are less than program limits.

Minnesota Supplemental Aid (MSA) - This program is a state funded program that provides a monthly cash supplement to people who are aged, blind or disabled and who receive federal Supplemental Security Income (SSI) benefits.

Group Residential Housing (GRH) - This program is a state-funded income supplement program that pays for room-and-board costs for low-income adults who have been placed in a licensed or registered setting with which a county human service agency has negotiated a monthly rate.

Refugee Cash Assistance (RCA) - This program coordinates services to assist refugees in making the transition to life in the United States. These services include resettlement and placement, cash and medical assistance, and employment and social services.

HEALTH CARE PROGRAMS

The following programs help low-income people with coverage for medical bills. How to Apply.

Medical Assistance (MA) - This program assists child under the age of 21, parents or relative caretakers of dependent children, pregnant women, people who are 65 or older and people who have a disability.

General Assistance Medical Care (GAMC) - This program serves low-income adults, ages 21-64, who have no dependent children under age 18 and who do not qualify for federal health care programs.

Other Health Care Programs - There are a variety of health care programs available to assist specific groups of people. These include persons in Nursing Homes, at-home care services, etc. the Department of Human Services web site is a good source of information on these programs.

EMERGENCY PROGRAMS

The emergency aid programs available in Ramsey County include:

Ramsey County Emergency Assistance (EA) - This program is for low-income families experiencing a financial crisis.

Emergency General Assistance (EGA) - This program primarily helps low-income individuals experiencing a financial crisis.

Emergency Minnesota Supplemental Aide (EMSA) - This program is only available to persons currently receiving MSA.

Emergency aide is usually provided in the form of specific payments for shelter or other emergency expenses, which cause a threat to the health or safety of the individual or family. People currently active on a program with Financial Assistance should call their caseworker to apply for emergency aide. People not currently on assistance with Ramsey County can call 266-4444 to get information on how to apply for emergency aide.

Emergency Shelter/Help for the Homeless - Ramsey County has a variety of services available to those who are homeless or in danger of becoming homeless. Intake services for homeless persons are coordinated through The Family Place.
